Saw Palmetto: The Most Studied Ingredient — With Mixed Results

Saw palmetto (Serenoa repens) is a palm native to the southeastern United States and the most extensively researched ingredient in this formula in the context of prostate health. It has been used in traditional medicine for urinary and reproductive complaints for centuries and is among the most widely sold herbal supplements for men.

Early clinical trials produced encouraging findings for symptom relief in men with benign prostatic hyperplasia (BPH). However, a large, well-designed randomized controlled trial published in the New England Journal of Medicine — the STEP study — enrolled 225 men and found that saw palmetto extract at standard doses did not improve lower urinary tract symptoms more than placebo [1]. A subsequent Cochrane systematic review of 32 trials involving more than 5,000 men found that saw palmetto produced no significant improvement in urinary flow or symptom scores compared with placebo [2].

These are significant findings that deserve to be clearly stated. The weight of current evidence does not support saw palmetto as an effective treatment for BPH symptoms, and Prostadine does not make that claim. The ingredient has a strong traditional use history and a favorable safety profile; the evidence for measurable clinical benefit in the general adult male population is weaker than its marketing reputation often suggests.

Pomegranate Extract: Preliminary Evidence, Active Research

Pomegranate (Punica granatum) contains ellagitannins and other polyphenols with antioxidant properties. Research interest in pomegranate and prostate health has been active, stemming in part from a small pilot study at UCLA that found pomegranate juice consumption was associated with a slowing of PSA doubling time in men who had been treated for prostate cancer [3].
